- Must be uninsured or currently enrolled in Medicaid
- Must be receiving pediatric care at Freedom Family Practice
If your child does not meet these criteria, we will refer you to the Polk County Health Department or another appropriate community resource where you can obtain vaccinations at low or no cost.
Respecting Family Choice and Medical Freedom
We understand that not all families choose to vaccinate. While we support public health guidelines, Freedom Family Practice does not force vaccines on any family. However, if you opt not to vaccinate your child, we do require the following:
- A documented medical or religious exemption
- A signed in-office waiver that acknowledges:
